illnesses or undergoes specified surgery, as set out below.

Cancer: The uncontrolled
growth or spread of malignant cells and the invasion of
tissue.

Heart attack: The death of a
portion of a heart muscle is a result of inadequate blood
supply |

Multiple Sclerosis: A
disease of the brain and spinal cord with impairment of motor and
sensory function which may lead to tremors, double vision or
paralysis |

Stroke: Insufficient supply
of blood to part of the brain producing permanent damage to the
nervous system |

Kidney Failure: Chronic
irreversible failure of both kidneys which requires either
transplantation or continuing dialysis. |

Major Heart Surgery: By pass
surgery to correct the narrowing or blockage of one or more
coronary.
Arteries:

Permanent inability to follow any gainful occupation due to
